The Community Health Needs Assessment (CHNA) is vital to the structure and implementation of health work in Shawnee county. The survey results provide community-informed data that is the foundation for creating the Community Health Improvement Plan (CHIP), a plan guiding social determinants of health work for organizations across the county.
We're looking for volunteers to hand out flyers promoting the survey, paper copies of the survey, and potentially gift cards for those who take the survey at local mobile food distributions. Volunteers will arrive at food distribution sites about 30 minutes before they begin, tell those participating in the food distribution about the survey and offer them a paper copy. Paper copies of the survey should then be gathered as participants get their food in the distribution line. Surveys will then be turned in to someone on the CHNA leadership team.
